2. Fill out the application for ColiCare

Applying for ColiCare is fast and easy! There's no paperwork required besides one simple form which can be completed online or over the phone. Once your application is submitted we will get back to you within one to two business days.
In order to obtain ColiCare coverage, your horse must meet the following eligibility requirements:
  • Reside in the United States
  • Be weaned and at least 5 months old
  • Be under the direct care of a licensed vet
  • May not have had previous colic/abdominal surgery, a history of chronic colic, or an incidence of colic in the past twelve months*
* Previous colic/abdominal surgery is defined as any previous surgical procedure to a horse's abdomen. We define colic in the past 12 months as any veterinarian-attended colic episode in the past 12 months. If the veterinarian was called to attend the horse by the owner or person with care, custody and control of the horse, this is considered a colic episode regardless of whether the attending veterinarian physically examined, diagnosed, or treated the horse. Lastly, chronic colic is defined as a horse experiencing 3 or more separate episodes of veterinarian-attended colic within any 12 month period.

3. Meet The Annual Wellness Requirements

Once enrolled, simply keep supporting your horse's digestive health with a ColiCare eligible product in SmartPaks and ensure your horse receives these basic wellness services every 12 months. There is no need to re-apply each year, and we don't require verification of the Annual Wellness Requirements unless you need to file a claim.

PROGRAM BENEFITS: Horses enrolled in and in compliance with the SmartPak ColiCare Program ("Eligible Horses") will be eligible for reimbursement from SmartPak Equine, LLC ("SmartPak") for up to $15,000 for colic surgery assistance fees ("Surgical Reimbursement").

ELIGIBILITY REQUIREMENTS: To be an Eligible Horse, a horse must be accepted into the SmartPak ColiCare Program. To be accepted, the horse's owner or the horse-owner's agent (who hereby represents that such agent has the authority to legally bind the owner) (collectively, "Customer") must have completed the SmartPak ColiCare Program Enrollment Form and have received confirmation that the horse has been accepted into the ColiCare Program ("Official Enrollment Notification")-see Program Requirements below.

To be an Eligible Horse, a horse must reside in the United States and be under the direct care of a licensed veterinarian ("Attending Veterinarian"). An Eligible Horse must be at least five months of age (and fully weaned).

A horse is not eligible for enrollment if it has had

  1. Previous colic/abdominal surgery-defined as any previous surgical procedure to a horse's abdomen;
  2. History of chronic colic-defined as a horse experiencing three or more episodes of veterinarian-attended colic within any twelve month period; or
  3. Experienced colic in the twelve months prior to applying-defined as any veterinarian-attended colic episode, regardless of whether the veterinarian treated the horse.

PROGRAM REQUIREMENTS: An Eligible Horse must meet the following Program Requirements to be in compliance with the ColiCare Program and eligible for Surgical Reimbursement:

  1. Daily administration of the maintenance dose (defined in table below) of SmartDigest Ultra, SmartDigest Ultra Pellets, SmartGI Ultra Pellets, SmartCombo Ultra Pellets, SmartCombo Senior Ultra Pellets, SmartCombo Ultimate Pellets, or SmartR&R Pellets ("ColiCare-Eligible Products") in the SmartPak supplement feeding system ("SmartPaks") for a minimum of twenty-eight consecutive calendar days (defined as a minimum of twenty-eight days from the shipment date) after acceptance, provided that an eligible dose of a ColiCare-Eligible Product must have shipped to Customer within twenty-eight calendar days of applying for the ColiCare Program.
  2. Documentation of at least one physical examination by Attending Veterinarian each year and compliance with all of Attending Veterinarian's recommendations (both as a result of any physical examinations or other recommendations of Attending Veterinarian),
  3. Documentation of at least one dental examination by Attending Veterinarian each year and administration of dental treatment(s) as recommended by such examination,
  4. Documentation of annual vaccinations administered by Attending Veterinarian of core vaccinations-such as those recommended by the American Association of Equine Practitioners (AAEP): Tetanus, EEE/WEE, WNV, and Rabies, as well as any additional vaccinations as recommended by such Attending Veterinarian, and
  5. Documentation of participation in a deworming program overseen by the Attending Veterinarian that includes at least one fecal egg count test and at least two deworming administrations each year.

REIMBURSEMENT: Surgical Reimbursement is limited to $15,000 over the lifetime of an Eligible Horse and is limited to Eligible Expenses incurred while a horse is an Eligible Horse. Any fees that exceed $15,000 are the sole responsibility of Customer. Expenditures eligible for Surgical Reimbursement ("Eligible Expenses") are only those costs incurred by the horse's owner or horse-owner's agent for a surgical colic incident, namely:

  1. Preoperative care incurred at a veterinary surgical facility,
  2. Surgery exclusive to the digestive tract incurred at a veterinary surgical facility,
  3. Postoperative care incurred at a veterinary surgical facility, and
  4. Costs reimbursed by a third-party are not eligible.

To apply and receive Surgical Reimbursement, the Customer must:

  1. Have received from SmartPak an Official Enrollment Notification,
  2. Have administered a ColiCare-Eligible Product at the maintenance dose in SmartPaks to the Eligible Horse for a minimum of twenty- eight days and for the entire time since acceptance,
  3. Notify SmartPak of surgery within fourteen days,
  4. Have Attending Veterinarian complete and send the ColiCare Program Compliance Affidavit to SmartPak, within fourteen days of the date of discharge from the veterinary surgical facility,
  5. Have the veterinary surgical facility complete and send the Surgical Reimbursement Form within fourteen days of the date of discharge from the veterinary surgical facility, and
  6. Have paid all costs associated with the colic surgery prior to submission and approval of documentation for reimbursement from SmartPak.

Only the Customer may apply for Surgical Reimbursement. Upon determination by SmartPak to complete Surgical Reimbursement, a check for the applicable amount will be issued in the name shown on the receipt from the Attending Veterinarian as the payor for the costs and delivered to an address provided by Customer.

LIMITED LIABILITY: SmartPak assumes no responsibility or liability for the rendering of services or products not under SmartPak's control. Compliance with the terms of this agreement does not constitute a guarantee that an Eligible Horse will not colic. This program is not intended to be a substitute for insurance coverage. Compliance with the terms of the ColiCare Program does not constitute an insurance policy.

TERMINATION: SmartPak may revise or cancel this program upon ninety (90) days notification to Customers with Eligible Horses. Your continued participation in the ColiCare program constitutes your acceptance of any such changes. Notification may be by any method used by SmartPak to routinely communicate with Customers with Eligible Horses. SmartPak reserves the right at its sole discretion to disqualify any horse, Customer, veterinarian, or veterinary surgeon from the program.

Best of all, they are delivered automatically every month, so you never have to worry about running out.
Q: Do I need to feed a certain amount of my ColiCare eligible product?
A: All horses over 500 pounds must receive at least the maintenance dose of any ColiCare eligible product, while all horses under 500 pounds must receive at least half the maintenance dose.
Q: If my horse is enrolled in ColiCare, can I delay my shipment?
A: Yes, you can delay your SmartPak up to 14 days and your horse will remain enrolled. If your SmartPak is delayed more than 14 days, your horse will lapse from the program and no longer be eligible for colic surgery reimbursement. However, any horse that has lapsed is allowed to re-apply for the program.
Q: Can I enroll my horse if I order in buckets?
A: For a horse to be eligible for ColiCare they must receive one of the ColiCare eligible products in SmartPaks so we can ensure your horse gets their supplements on a daily, uninterrupted basis.

Q: How do I enroll in ColiCare?
A: Enrolling is easy! You enroll online by visiting the ColiCare Enrollment Page, or give us a call at 1-800-461-8898 and we'll get you started in less than 5 minutes.
Q: After I enroll, how long before I know if my horse was accepted?
A: Once we have received your completed registration form, you should hear from us within 3 business days.
Q: If I just added a ColiCare eligible product into my SmartPak, when can I enroll?
A: Right away! You can enroll by visiting the ColiCare Enrollment Page or by calling us at 1-800-461-8898.
Q: What type of paperwork do I need to submit to enroll?
A: No paperwork or documentation is needed to enroll except for the application form! You only need to submit proof that your horse has met the Annual Wellness Requirements if you file a colic surgery claim. Otherwise simply keep your horse's records in a safe and easily accessible place, should you need them.
Q: Do I have to complete the Annual Wellness Requirements before enrolling in ColiCare?
A: While we don't require documentation of the Annual Wellness Requirements at the time of enrollment, should your horse undergo colic surgery, we will require verification that your horse has met the Annual Wellness Requirements in the 12 months prior to your claim.
Q: Why aren't SmartGut and SmartGut Ultra included in the ColiCare program?
A: SmartGut and SmartGut Ultra are our targeted gastric health formulas designed specifically to support the stomach. The ColiCare eligible products are the only supplements included in the ColiCare program because they are designed to support the health and balance of the horse's hindgut. If you are looking to support your horse's stomach and hindgut, we recommend SmartGI™ Ultra, which contains the active ingredients of SmartGut Ultra together with SmartDigest Ultra, and thus is eligible for the ColiCare program.
Q: Is there a limit to the number of horses I can have on ColiCare?
A: No; as long as each individual horse meets all eligibility requirements, owners may enroll as many horses as they would like.
Q: Is my horse's age a factor for eligibility?
A: We welcome senior horses into the ColiCare program! As long as your horse is at least 5 months old and weaned, the eligibility requirement for age is met. There is no upper age limit!
Q: What happens if my enrolled horse has a claim approved for less than $15,000?
A: ColiCare offers a reimbursement of up to $15,000, so long as your horse remains enrolled in the ColiCare program. Provided that you also continue to meet the Annual Wellness Requirements, the remaining balance will be available to you, should you need it for a second colic surgery.
Q: What types of colic does this program cover?
A: As long as your horse has met all of the Eligibility and Annual Wellness Requirements, ColiCare offers up to $15,000 in colic surgery reimbursement regardless of the type of surgical colic your horse experiences.
Q: How do you define previous colic/abdominal surgery, colic in the past 12 months and chronic colic?
A: Previous colic/abdominal surgery is defined as any previous surgical procedure to a horse's abdomen. We define colic in the past 12 months as any veterinarian-attended colic episode in the past 12 months. Q: What if my horse is leased out or managed by my barn manager or trainer?
A: Leased horses are absolutely welcome into the ColiCare Program; however the horse owner must be the one to enroll the horse.
Q: Are horses outside of the United States eligible for enrollment in ColiCare?
A: No, the horse must reside in the United States to be eligible for enrollment in the ColiCare program.

Q: Is my veterinarian required to provide dental care?
A: If your veterinarian offers dental care, perfect! We realize, however, that not every veterinarian offers dental care services. In these cases it is important that your veterinarian perform an annual exam of your horse's mouth, and that any additional dental care he or she recommends be performed by a trained equine professional.
Q: Can I administer my own vaccinations?
A: We require that your veterinarian administer all of your horse's vaccinations. It is important to have your veterinarian involved in the selection of appropriate vaccines for your individual horse, and proper vaccine handling is also critical to their effectiveness.
Q: Does my veterinarian have to perform the annual fecal test?
A: If your veterinarian offers fecal testing, excellent! However, if your veterinarian does not provide this service (fecal egg per gram test), consider purchasing an Equine Fecal Test Kit from SmartPak to meet this Annual Wellness Requirement.
Q: Do I have to buy dewormer from my vet?
A: No, you can purchase dewormer from your veterinarian or any equine retailer. To meet this Annual Wellness Requirement you must be working closely with your veterinarian to develop an annual deworming program that includes at least one fecal test and two deworming administrations.
Q: Does the veterinarian that performs my horse's colic surgery need to have any particular credentials?
A: The only credentials required of the veterinarian that performs the colic surgery is that he or she be licensed in the state in which the surgery is being performed.

Q: Does ColiCare offer reimbursement for medical colic episodes?
A: ColiCare is a colic surgery reimbursement program, and therefore only covers costs associated with colic surgery (surgery related specifically to the digestive tract) incurred at a veterinary surgical facility. It does not cover expenses associated with a non-surgical colic.
Q: Does ColiCare work with insurance?
A: Yes, ColiCare is fully compatible with all equine insurance policies. In the event of an approved colic surgery claim, SmartPak will reimburse the horse owner up to $15,000, independent of any insurance or other reimbursement program associated with the horse. ColiCare makes a great complement to equine insurance policies, as a way to provide additional financial coverage.
Q: Can you confirm if my claim will be approved before making the decision to go to surgery?
A: You can always contact us to confirm your horse's enrollment status in the ColiCare program at any time, and this information is also available on the ColiCare Management page when you log in to your account. We are not able to guarantee that any colic surgery claim will be approved until we have received and reviewed all of the required paperwork pertaining to that claim after the surgery is performed.
